Service Dog Assessment

If you are considering the possibility of acquiring a service dog or are curious about the process involved, we invite you to complete this questionnaire. By providing us with some essential information, we can better understand your needs and guide you through the steps towards potentially welcoming a serivce dog into your life. Lets embark on this journey together and explore the transformative impact a service dog can have on your daily living.

Vaccination Requirements

To prioritize the safety and well-being of all dogs participating in our training programs, we kindly ask that all canine attendees meet the following vaccination requirements:

  • Rabies: All dogs must be up to date on their rabies vaccination as required by law.

  • Distemper/Parvo: Dogs should be current on their distemper and parvovirus vaccinations to prevent the  spread of these highly contagious diseases.

  • Bordetella: A current bordetella (kennel cough) vaccination is recommended to reduce the risk of respiratory infections in group settings.

  • In addition to the vaccination requirements, we also ask that all dogs provide a negative fecal test conducted within the last 6 months. This test helps ensure that participating dogs are free from any internal parasites that could potentially be transmitted to other dogs during training sessions.

  • Don’t have a fecal test? No worries! We now offer this service to our clients. Simply inform us, and we’ll make sure you’re all ready to go.
